Recording Release Agreement

If you would like to receive an archival copy of your recital video for personal/private use, please review the current Recording Release Agreement linked here. If you agree to the terms as outlined, you may either: 

  • Print and sign the form. Once completed drop that off in the Russell Hall main office (RSL 115) or to Caroline Francis in GBPAC 76. 
  • Complete the webform below to sign the agreement electronically. 
  • Once your signed agreement is on file, you may request videos from Tom.Barry@uni.edu.
