Instrument Rental Form

Complete this form if you are checking an instrument out for the semester. Please read the terms of the agreement at the end of the form carefully, and note that a $45 charge will be added to your U-bill.

All instruments and accompanying equipment are property of the University of Northern Iowa, and may be re-claimed by the director or their designee at any time. 

University-owned instruments may be issued to registered UNI students who are: 

  1. Participating in School of Music sponsored activities.
  2. Registered for School of Music courses that require the use of an instrument. 

All university-owned instruments must be returned in acceptable condition and by the last scheduled class meeting of the semester in which the instrument was issued. 

University-owned instruments are NOT to be taken from the UNI campus without the express permission of the director or designee unless the instrument is to be used in a School of Music sponsored activity. 

All responsibility for the care and protection of university-owned instruments rests with the person to whom the instrument has been issued until such time as the instrument is returned to the UNI School of Music. If an instrument if lost, stolen or damaged in any way, the student to whom the instrument has been issued must assume full financial responsibility. 

If a student to whom a university-owned instrument has been issued withdraws from class for any reason, this contract will be considered terminated and any UNI instrument(s) and equipment in the student's possession must be returned to the School of Music immediately. 

By submitting this form, you agree that you have read the above statements of policy regarding the use of music instruments owned by the University of Northern Iowa School of Music, you understand the policies fully, and you agree to comply with all policies. You also agree to accept a $45 charge added you your U-bill per semester.

Instrument Exchange Form

Complete this form if you are exchanging your rented instrument for a different instrument. 

Please note: you must complete the instrument return form above FIRST before a new instrument can be issued! Once the originally rented instrument (and any additional equipment that was issued) has been returned and the return form is complete, proceed to complete the Instrument Exchange Form. As long as the original instrument has been returned, there will be no charge for the instrument exchange.
